• Securing good nutrition, delivered meals, grocery shopping, running basic errands
• Identifying contractors for home maintenance and to improve home safety and security 
• Immediate housing needs, developing plans for future housing options

Our interdisciplinary team, led by a geriatric care manager, facilitates an integrated plan of assessments, service coordination, referrals, advocacy and direct care.

Solutions care management is especially beneficial for people with complex problems, chronic medical illnesses or memory disorders such as Alzheimer’s disease; those who require close oversight and coordination of care; and those without family close by.

The first 50-minute consultation about someone 60 years and older is free. Our Solutions Care Managers will listen to your needs and develop an action plan – all as part of our efforts to help individuals and their families maneuver the aging process with dignity and quality.
